06-08-2014: Launch of Hull 2020 marks new era of partnership for public services in Hull

Hull 2020 launches today, with a vision of a brighter future for our city, shaped by public services working together as a single system by 2020.

The nine public service organisations that make up Hull 2020 are committed to transforming the way we all work to enable the people of Hull to improve their own health and wellbeing and to achieve their aspirations for the future.

The Hull 2020 programme is led and delivered by:

Yorkshire Ambulance Service
Humberside Police
Humberside Fire and Rescue Service
Hull and East Yorkshire Hospitals NHS Trust

Hull 2020 aims to radically change the way services are delivered so that people will experience services that fit their lifestyles and needs, designed in partnership with them. Public organisations will work closely together to deliver services that make sense, without the waste and inefficiency that can sometimes frustrate people.

As part of the transformation programme five work streams have been developed to bring about the significant change needed to ensure sustainable high quality services are delivered by 2020. 

Workstream one - Frailty and Isolation

Lead: Steve Topham – Humberside Fire and Rescue

“By 2020 we can make difference if communities work together to keep themselves safe.”

A range of services are provided across Hull to meet the needs of some of the most vulnerable in society, specifically this includes those such as dementia patients, elderly people, those with mental health and learning disabilities.  This work stream is tasked with co-ordinating these services to meet common needs. 

Year one priorities include:

  • Winter Strategy – Cold Alarms
  • Telehealth and Telecare Review
  • Review of Falls Service (Riverside initially)

 

Workstream two - Children and Young People

Lead: Mil Vasic, Hull City Council and Julia Mizon, Hull CCG

“By 2020 we can make difference if children have the best start in life.”

This work stream has a strengthened and clear focus on children and young people as the ‘next generation’. The recent review of maternity services and the development of a five-year maternity services strategy has had a significant impact and there is an opportunity to build upon this and align services and activities to improve the lives and outcomes of the children and young people of Hull. 

Year one priorities include:

  • Paediatric unplanned care pathway review
  • Children and adolescent mental health pathway review
  • Educational attainment and aspiration

Workstream three - Life Choices

Scott Young – Humberside Police

 “By 2020 we can make difference if people take greater responsibility for their own health and wellbeing.”

There is a wide range of agencies currently delivering initiatives to continue to reduce the numbers of people who smoke, help more people reach and maintain a healthy weight, encourage responsible drinking and work together with the police to reduce crime and keep safe. This work stream is intended to align to give greater impact across the city. 

Year one workstream priorities include:

•       Community Safety Review led by Humberside Police

•       Reducing reoffending/restorative practice

•       Priority Families Programme – linked initially to supporting young families in Riverside area

•       Development of public service navigation portal

Workstream four - Planned Interventions

Lead – Julia Mizon, Hull CCG

“By 2020 we can make difference if people in Hull feel positive about their future.”

There are opportunities to transform pathways and management of patient care in high volume (outpatient) specialties and give patients a greater choice of services to improve their experience of assessment, diagnosis and treatment.  The ideal model is where patients no longer wait to be seen but are be able to choose when and where they receive their care at a time that is convenient to them.

A key priority is to achieve a shift of activity out of hospital by having more specialist services in the community supported by primary and community health and care services that are accessible, understandable, effective, and tailored to meet local and individual needs.

Year one priorities include:

  • Improving direct access diagnostics
  • End of life care pathway review to further improve choice
  • Review telecare provision
  • Review of adult mental health services

Workstream five  - Urgent Interventions

Lead: Erica Daley, Hull CCG

 

 “By 2020 we can make difference if people know how and where to get help if they need it.”

This workstream brings together all elements of healthcare that respond with urgent and emergency care services.  These systems and processes need to be able to work 365 days a year to avoid the crises we have seen over the past few years.  Systemised delivery of urgent intervention will ensure a more managed approach to urgent care and mitigate the risk of surges of activity impacting on capacity across health and social care.

Year one priorities include:

  • Enhanced collaboration with Yorkshire Ambulance Service
  • Develop new pathways for older people attending hospital and people with long term conditions
  • Primary care services in the Emergency Department

Each organisation will make sure their own strategies and plans to take their own business forward will support the Hull 2020 strategy, with the overall programme is driven by the Hull 2020 Board.

“Working in partnership is not new. What is different about Hull 2020 is the number of organisations that have committed to making this a success and the fact we already have a strong programme in place to enable us to move towards - with the support of our communities – achieving the vision set out in the Hull 2020 strategy.

“We understand that to succeed we need to inspire local communities so we can take them with us on the journey. This means having conversations with people across the city about what really matters to them. Hull 2020 is already under way and we need local people and organisations to get involved to make the services that people rely on in this city are designed with them in mind.”
